One condition - AudioSwitcher must enabled;
Just tested. It seems to work almost good already. I have simultaneously audio output but the only problem is that I only get the core signal on my AVR (So no DTS-MA but only DTS). My D-Box decoder receives the (core) signal that it needs perfect.

ps. On the second picture you see my D-Box software that works perfect with this version of MPC-BE! (Optical Sync notification which means that it works)

P.S. I think i understand what happened - if D-Box decoder support only DTS(core), audio decoder failed from DTS-HD bitstream to DTS core only. As you see - use only one audio decoder.

My AVR supports DTS, DD, True-HD, DTS-MA etc etc. I have the newest topmodel from Onkyo.
Settings in LAV are the settings I always use: checkboxes with Dolby Digital (AC-3), Doly Digital Plus (E-AC3), Dolby TrueHD, DTS and DTS-HD.
I can tell you that with this MPC-HC version that has dual-audio output implemented it works http://en.sourceforge.jp/projects/sfnet_mpc-hc-dboite/. Also with that version it is not needed to check the BUILD-IN AUDIO SWITCHER option. It works with and without this option. (ps. With 'IT WORKS' i mean that i the core DTS track is send to my d-box decoder and the DTS-MA track is send to my AVR)

You ARE right that the D-Box decoder ONLY works when it receives the UNALTERED CORE signal, so the core dts or Dolby Digital track. So I use my on-board spdif audio 'card' for the D-Box decoder (you can see this as soundcard 1) and motherboards HDMI output (you can see this as soundcard 2) for my AVR (which passes through the picture and hopefully will pass through the lossless DTA-MA or True-HD track soon :))

I have more information for you. (I m at work and will be back home within a few hours so I can give you better information that you might need):
With the MPC-HC version made by a D-Box user http://en.sourceforge.jp/projects/sfnet_mpc-hc-dboite/ I did always see TWO times LAV Audio Decoder in the filter list when playing a movie. With the version you made you can see that I only see ONE lav Audio Decoder in the filter list when playing a movie.
Maybe this information is important for you?

Multiply AudioDecoder for one audio track it's a bad idea. I make - multiply output from audio decoder to different audio renderer.
My English is bad, so I can not explain all the reasons it is such an implementation. But - as is done in mpc-hc-dboite - bad, and does not always work.

What is your plan on making it work for me?

Seems good with the internal audio decoder at least. I finally set up my htpc properly 2 days ago to test it and haven't had sync problems yet. I'll try again with LAV and see what happens.

I also use MPC Audio Renderer + WASAPI Exclusive + internal audio decoder. Use bitstream output and via decoding to PCM, and also good, no A/V desync.

No planned yes use multiple audio decoder for dual audio support.
There are many nuances.
Example - how to be with external audio files ? If use LAVSplitter & only internal audio stream all will be ok, but if use internal splitter, use external audio files(dub) - we can't split audio decoders for each of audio streams.

You use madVR - it's use own OSD and it's settings can not be changed.

Getting up to 100 cpu
Any ideas as of why and how to fix it ?
Mpc-be x64 any of the latest versions
Version 1.0.3.1x64 works ok 7-10 %cpu
the only External filter is AC3 everything else is default

About subtitle - i do not understand what you want ... english is bad.
About MadVR drop frames - what filter you use. Try only with internal and what result ??

It's kind of tough to explain and I'm horrible at explaining things. The goal is to get the subtitles as close to the of the bottom of the video without disappearing. MPC puts one line subs where the last line would go with multiple lines, asking for it to go where the first line would go, near the bottom border. Subs also disappear in MPC when bottom margin is negative if there is no bottom border, instead they should show on the video. MPC would need an option to show subtitles off the video (if possible) which could be controlled by the current top and bottom options.
Same problem with internal filters, backbuffer keeps running out in MPC so dropped frames. In potplayer decoder queue drops a bit but backbuffer stays full and no dropped frames.

Guys, you can configure mpc-be so he always open videos in 1280x720 resolution? No matter what the resolution of the video, I just want to mpc-be open always in 1280x720. I'm trying to understand the logic que applies to the Auto setting under Auto Fit Zoom in MPC. I do not get it. With it set as default, some videos are resized to somewhere in between 100% & 200% size, while others are resized to greater than 200% size of the original video. I can not figure out the logic that's used by the program to determine what it resizes to. Does anyone know how this setting works or what it does exactly? Is to configure it manually?

Still did not work. I tested some videos here of 640x480, the maximum that remained was 960x720. Before they were all in 1280x720 when I used the autofit option, but I had to uninstall mpc is corrupted by yesterday. After that, I could not use at 720p, now the autofit choose on their own.

One detail I realized I forgot to mention while testing this is that you have to have the "remember last window position" option enabled for it to work. It's working fine on my computer that way.
You have to completely uncheck the auto-zoom option though, not use autofit(which I have no idea what it actually does).

I did what you said, but did not solve the problem. For example, videos with 640x480, 1280x720 usually climbed before with autofit on, now with the hint that you taught me, the mpc scale 1280x720, but the video only opens at 960x720 with black borders on the sides.

You can try this: View -> Video Frame -> uncheck Keep Aspect Ratio (you can even choose Override Aspect Ratio to 16:9), but do you really want to watch distorted video, because 640x480 video have 4:3 AR.

That's normal and the intended behavior since most wouldn't want to watch a stretched video.
If that's really what you want, do what the_weirdo said and it should stretch it, but it seems that it doesn't.
I tested it and my 4:3 video stays in 4:3 even with Keep aspect ratio unchecked and Override aspect ratio to 16/9 so it seem to be a bug.
In fact, none of the options under Video Frame seem to have any effect on the video under those conditions.
Edit: OK, did a bit more testing and it seems that EVR can't change the size or the aspect ratio of the video(I forgot I still had it on since I last tested something). It works fine with EVR CP and madvr, but you have to check "Keep aspect ratio" for the "Override aspect ratio" to "16:9" to work which seems like a pretty odd behavior(MPCHC is the same), but at least it works.
