DV Date/Time To Subtitle

I'm putting together the DVD of my daughter's second year home videos from multiple MiniDV tapes. I want to extract the time/date from the DV source and convert it to a subtitle track on the DVD, so that I can enable it and see the date each clip on the DVD was taken. I capture and edit via Premiere and then feed the edited output to the CCE plugin. Is there an easy way to create the subtitle track?
