After encoding a movie with permanent subs ripped using VobSub 2.16, I found that they were appearing 2 seconds too early. Since the desynch was non-progressive, I inserted a "time offset: 2000" in the .idx file and then re-encoded, but there was no change. I even tried manually adding 2 seconds to each of the timestamps, but still no luck. How can I add a 2-second delay to all of the subtitles? Does the .sub file have to be recreated somehow?

You should be able to use SubResync and open your sub files and modify the first timestamp and it will adjust all the rest of the times and then you just save the idx/sub file.
