Adding custom subs to DVDs

How do I add custom subs to a DVD?

I have subs as either srt or sub and idx.

You can use SubtitleCreator.
Tools -> DVD authoring wizard.
If your sub/idx are multilingual, extract each substream first (SubtitleCreator can do that).
