Combining DVDs Into One: Retain Menus or Chapters?

If combining two or three DVDs into one, can the menus be retained, or can the chapters be retained?

If so, how so?

Yes, but the only program that does it easily is DVDRemake Pro (the regular DVDRemake can merge two DVDs):

It can be done manually with freeware, but you don't want to try:
