I use Yamb to extract video and audio streams.
Then use MeGUI "muxer/mp4 muxer" to mux them back together with the srt file.

EDIT: Also, from VSFilter docs
Code:
  mpeg2source("F:\From_hell\from_hell.d2v")
  TextSub("F:\From_hell\fh_ned.srt")

It seems Handbrake and Subler are also options: http://forum.videohelp.com/threads/356219-Getting-soft-subtitles-on-my-iPad-and-Nook.

I think you need to first convert the SRT to TTXT, read part "7) How can i mux Subtitles into MP4 files?" of this FAQ:
http://forum.doom9.org/showthread.php?t=62723

MP4Box can do it and you can use YAMB as a GUI:
