I just encoded a short sample with the commandline above and it produces file that crashes ffdshow. So there is a bug somewhere... The one diffrence is that my file starts playing and crashes in the middle (first bframe used as a reference?) I'll post the file if needed...
---EDIT---
Link: this crashes ffdshow (http://rapidshare.de/files/4180956/avc_movie.mp4.html)
---EDIT 2---
Just checked my file in mplayer, and it doesn't want to play although the raw .264 that was produced by the commandline is playable in mplayer...
---EDIT 3---
I switched the output in the commandline above to .mp4 and the file is playable both in mplayer and ffdshow. Now i'm confused...

how did you produce the .mp4 you linked to?
make sure you use latest versions of mp4box. the mp4box on rarewares is outdated
edit: also your file doesnt have a ctts atom necessary for b-frames

the .mp4 file you link to has a resolution of 16x16.
I know! It was produced by
MP4Box - GPAC version 0.3.0-DEV
and the source was this .264 file. (http://rapidshare.de/files/4181860/avc_movie.264.html)

Yeah... It was mp4box problem (my mp4box was nearly 2 months old). Sorry, my mistake :(
