Hi,
I have a DVD with 2 audio streams on it and I would now like to re-author and add a third audio stream to the main movie on that DVD. What is the easiest way to achieve this? The audio stream is in AC3 format and of correct length.

Demux you vobs, use an authoring program to put everything back together including the extra audio. Update your original ifo with the info from the reauthored one using ifoupdate/batchupdateifo.
