Cuttermaran (http://www.cuttermaran.de/frameset.htm) + TMPGEnc (http://www.tmpgenc.net/e_main.html) or Cuttermaran (http://www.cuttermaran.de/frameset.htm) + CuttyEnc (http://www.psimarron.net/Projects/DVD/CuttyEnc/).

You can try DVTool, but sometimes it produced unplayable files. Other what come in mind is avidemux2, but I never tried it for cutting. More info:http://forum.doom9.org/showthread.php?s=&threadid=81907

I haven't used it myself so I'm not sure but maybe virtualdub-mpeg2 (http://fcchandler.home.comcast.net/stable/) does what you want. :)

virtualdub-mpeg2 let you open mpeg2 files directly with virtualdub instead of using DVD2AVI and Avisynth.
