the .mp4 Container only use made for MPEG codecs. reals aac can you put in .mp4.
possible content for .mp4:
MPEG1,2,4 Audio and Video

You can place private streams (nonMPEG content) in MP4. whether this is useful or advisable is another question.

"raac" (Real AAC) and "racp" (Real AAC Plus) are mpeg4 and you can put them in mp4 or m4a
other two ("sipr" and "cook") Real codecs are NOT mpeg and there is no use of puting them in mp4

or use: rm -> mkv -> aac -> mp4

real (thats the name of the company and not a codecs name) provides a whole bunch of audio codecs under "realaudio" which are all not mpeg-4 compliant, except their latest one, which is aac compliant (which is a part of mpeg-4) and can therefore also be placed in .mp4
