How can I insert a JPEG image of a CD/DVD cover into a Matroska file so that media players and collection management tools will be able to display it?

You use mkvmerge GUI (in the mkvtoolnix package from Mosu), you open the second tab "attachments" and "add" the picture. Then you need to change that file's MIME type to "image/jpeg" (or how that exactly is called) lower down on the same tab. Then you just mux your file like usually.
In case you want to add to a already muxed mkv, you can always open the mkv and only add the attachment and remux to a new file. The present streams and settings should be perfectly preserved.
You can make sure the picture is attached properly with the explorer extension (matroska props by Jcsston). Go to its "matroska attachments" tab, if the pictures are listed and previewed correctly you have done well.
The real question is rather, what applications currently allow you to view these attachments..

At the moment I don't think there's any application (media player) that can show attached cover/images in mkv.
