I've finally made it. I was backing up Resrvoir Dogs - Spec.Ed and there are CC in it in English. Subtitles are only in Spanish. If you do it with RB you lose CC. Problems were that movie is seamles branching for critics and director's comentaries.

SO this is what I did:
1. Used DVD Stripper to rip it and get rid of some extras.
2. Compressed all other extras - no movie compression with DVD Shrink.
3. Used Vobsub to extract CC subtitles.
4. Used SRT2SUP to convert them to .sup file (Ifoedit authoring).
5. Encode result of the DVD Shrink with DVD-RB.
6. Demuxed all streams with Vobedit including spanish subtitle.
7. Replaced spanish subtitle with english one (use same name VTS_01_0.20.sup).
7a. Get CellTimes with Ifoedit from the DVD Shrink result VTS_01_0.IFO.
8. Re-muxed it with Rejig 0.5d (0.5e was crashing on my machine). Inserted all the audio stremas that I wanted, subtitles and cell times.
9. Copied original IFOs from DVD Shrink and all other VOBs missing (VTS_01_0, VTS-02_XX etc.)
10. Corrected VTS sectors and copied subtitle colour from the original IFOs. Edited VTS_01_0.IFO to show English substream instead of Spanish.
11. With MenyEdit disabled dead buttons.
12. Verified that everything is working.
13. Burned the DVD.


Tedious job, but subtitles are wortking fine, only drawback is that meny for subtitles control is disabled and say Spanish. I use remote to turn subtitles ON/OFF.

That's a clever work-around.

I thought there were some things about captions that couldn't be directly converted to subs. Maybe position on the screen and line breaks?

If I had a caption only disk, such as Ten Commandments which I did yesterday, my procedure has been to use Shrink or the manual CCE method to retain captions.

but I didn't have Scenarist, so when I authored with Ifoedit, got choppy stream for NTSC. I never tried with Rejig.
I am not sure that CC is separate stream or inserted as a line 21 of the mpgeg stream. But vobsub detects it and you can get text file output. Just watched Reservoir dogs backup last night and CC is synchronized, except for some parts are missing - probably wasn't extracted correctly. Anyways it is only few sentences here and there.
