I just did a small test with short clip
using fastest mode 1-pass with default bitrate 780.
The result is a very big size, over than 5x the normal size.
Is that normal ?

Noticed that even with Standard 1-pass at 200 kbps. Unusually large files. Why?

Yes it is normal for fastest mode. Because the majority of blocks are intra-coded the bit spend is extremely high and its unlikey the rate control will be able to hit your target bitrate.
See page 48 of the guide.
IMHO you really don't want to be using fastest mode for anything except video capture (in combination with 1-Pass, Quality-based mode) where you don't care too much about the file size (e.g. if you were going to re-encode later perhaps).
