Is it Region 1 NTSC?.
If so, try reviewing @2COOL cheetsheets for similar dual-format titles to get started, or if you are lucky a new cheetsheet for this DVD from da man himself
If you run into problem at the steps, post up and you will have help. Give it a try and make a good learning exercise.

Cool, thanks! Do you think this will work for all movies with extended versions, or is this specific to Identity?

This cheat sheet is specific to this regional DVD. The goal of my cheat sheets is for people to be aware of what to look for in the IFO to hopefully help them on future backups.
Learning by examples. ;)
