Authoring MPEG-4 video with MicroDVDPlayer

I like to have chapters in my MPEG-4 XviD encodes.

My questions are:
1. Is MicroDVDPlayer the only player that can handle chapters?
2. Are there other players that can handle chapters in MPEG-4? If yes, can HW players, like Kiss DP-450, handle these chapters too?
3. Are there other tools to create chapters respectively the .ini file?
4. Is there a standard for MPEG-4 authoring? Or have we to wait?
