To me, the Mode2 CD Maker seems to work great. Is it as "unsafe" as the XCD guide makes it seem? And are there A/V format combos that should be avoided at all cost?

It all depends on the error correction capabilities of the container. So, currently, i.e. until matroska has cached up, ogm is first choice because it has buit in error correction.
