Sync Problems

Can Anyone help me out here please, ive done loads of ifo edit m2v remuxes with success they play perfectly but every now and again i do a dvd that plays perfectly but the audio and video are way out of sync, i dont know why unless ifo edit doesnt pick up on something, has anyone else had this problem.

Can someone help me please ?
