I’ve got a DVD with English, Francais and Espanol subs
I want to keep the English and replaces one of the two others with a 4th subtitle.... Is it possible?
What do I need to do this (If possible)

I would say:
just demux your title with doitfast4u.
Reauthor it with a program that supports subtitles (E.G. MAESTRO OR SCENARIST).
In stead of adding all your original subtitles add the one you want to use and recompile it. That should be all I think.

Will I be able to use the org. menus? Let’s say I still get the Eng. subs by selecting English and my new subs by pressing lets say French. And then I can disable the third button. (I know how to disable buttons in the menu.)

I was assuming your dvd is 4,7 GB or smaller, so no additional encoding etc is necessary.
Ofcourse you can use your original menu etc.
If your dvd is 4,7 GB:
Demux your DVD using doitfast4u (make sure to rip your english subtitles too).
Import your video and audio in scenarist/maestro.
Import your subtitles (original and new).
Compile your project
Use ifoupdate to update the original ifo (don't forget get vts sectors)
For your unused subtitle you can either disable the menubutton or edit your ifo and set the stream value to 0 (under pgcity). Disabling the button is an nice option ofcourse.
About the rauthoring part you can find a lot of guides ofcourse.
