MPEG > Modulation > H.232 ?
Modulated isn't a quantization for itself. It's only a method that Koepi came up with that chooses between H.263 and MEPG quantization, depending on the quantizer used for that particular frame.
Someone please correct me if i am wrong.

You're right, it's no "level of it's own".
But it _can_ be sharper as pure .h263 quant type, if the quantizer range allow some frames with quantizer < 4.

Would MPEG 1st pass + MPEG 2nd pass results in all ALL MPEG final frames?
Basically, what would be the final results of the following combinations:
(1st pass) (2nd pass)
MPEG + MPEG
MPEG + H.xxx
H.xxx + Mpeg
Mod + Mpeg
Mpeg + Mod
Mod + Mpeg
Mod + H.xxx
H.xxx + Mod
Mod + Mod
If I want maximum sharpness should I choose MPEG for BOTH passes then?

And then, if you still dare, ask that question with those combinations and that "result" again...
