I'm having a problem since a lot of time.
I've used vapoursynth on my personal computer for a lot of time without problem but since i don't know precisely when i've started having problem with indexing files.
I've always used lsmas for indexing with mvtools for denoising and fmtc for resizing [often together] and then lsmas started doing something strange [ https://github.com/VFR-maniac/L-SMASH-Works/issues/43 ], then flipped to ffms2 and after a while the problem happened again, the i waited for 2.22 and i was free from this problem again but then again i had this problem recently.
the issue is hard to track down, it mostly happen when non linear access is involved [denoising mostly] but i reproduced the issue even without non linear access.
This problem seems to happens mostly with BDs [demuxed using eac3to in mkv].
I'm now unable to use vapoursynth in any heavy weight script [the ones i use most].
