I've fallen out of touch with what's going on in the AVISynth world the last few years, so I'm not sure what the current state of things is. I don't see a lot of talk about denoisers here like I used to. I like MCTemporalDenoise, but it's very squirelly and unreliable under any MTMode except 5 and I don't have the patience to run it single threaded. Is there another denoiser/sharpener that is either considerably faster single threaded or that works properly multithreaded?

Don’t use MT if it is not reliable. Instead, you can split your video in several parts, encode them at once, and join them by remuxing.
