Avidemux will work in a pinch (and might be OK for just cutting commercials) but its interface is not all that specialised for heavy editing.
Back when I used to do that sort of thing I purchased VideoReDo which allows lossless (apart from the GOP you're editing) trimming of commercials in MPEG-2 streams and was pretty good with auto-detection.
Then you'd just feed the result through x264 later...

What kind of PDTV? While cutters for SDTV are like sand in the sea, few HDTV cutters are really available. Of course, the complexity of the PDTV is another issue - I like to have all streams cut (that is together with subtitles and audio streams).
