Playback problems

I'm using always-updated versions of MPC-HC (currently using JW32's builds), LAV filters, and madVR. Sometime over the past several months (not sure exactly when, but between the release of MPC-HC 1.6.5 and 1.6.6) I started having three playback issues which I've now had for about two months.

1. During playback of a long video, the screen flashes completely black momentarily before the video resumes. The audio doesn't skip. This happens roughly once every 40-60 minutes.

2. The mouse cursor disappears like it should in full screen mode, but reappears after a minute, and remains on-screen for the duration of the video.

3. MPC-HC sometimes remains a running process after closing.

A little bit more info: my settings mostly follow Niyawa's guide. My CPU is an Intel 3570k, my GPU is a Radeon HD 7950, the monitor is 1080p, and my PC is running Windows 8 x64. I happen to have reinstalled the OS at one point but that didn't fix any of the issues. I'm watching videos on my TV@23hz over HDMI, and madVR reports that there should be one frame drop every 35-40 minutes because the TV refresh rate isn't exactly 23.976hz.

It's odd because playback was trouble-free several months ago. Could anyone suggest the cause of the problems, or how I could fix them?

I do not have a solution, but the cause is obvious.
There is a short between the seat and the keyboard.
When playback works, STOP tinkering with it.

Well actually I'm not an imbecile, so the thought of rolling back my software and drivers had occurred to me. However that's a suboptimal solution. I could even go back from Windows 8 to Windows 7 by that logic because I have bugs in 8 that I didn't have in 7, but the new additions in 8 are too valuable to me to consider doing so. As for graphics drivers, I could roll back to December but there is absolutely no way; AMD drivers become monumentally better with every iteration. I also happen to find many of the recent madVR and LAV developments useful, so again, I'm not rolling those back because of these comparitively minor inconveniences. The only thing I could feasibly roll back is MPC-HC itself. I've tried older versions, but it doesn't seem to help.

I came here to ask for help hoping someone could actually identify these specific problems and offer fixes or workarounds. Thanks for your input but I will remain hopeful that someone actually knows something.

Edit: Solved. Newer MPC-HC versions close properly. The mouse cursor problem is a result of having a Chrome or Afterburner window open in the background. The black flash... is caused by AMD driver spikes between 2D and 3D clocks.
