hi
i have the same problem with some files
so i understand we must put FFFMS thread 0 to 1 in option
i tried and it s not working
[Error] [18/08/2013 16:24:05] avs [error]: FFVideoSource: Insanity detected: decoder returned an empty frame occurred while reading frame 2
no way to ignore this error ?
on this file it s frame 2 , so i use avs cutter to cut the 2 first frame but can't do that on all the files where is an error

Sometimes remuxing the source file into another container can help. ie if it's an AVI try remuxing it as an MKV first.
ffmsindex seems to have the occasional issue. Swapping the current version for an older one has fixed a similar problem for me in the past.
If you're basically just encoding the video from start to finish and not applying any complicated filtering (just using MeGUI's standard filtering) the simplest fix might be to use DirectShow to do the decoding instead. The easiest way to do that with MeGUI is to open the Script Creator from the Tools menu, then use it to open the video in question. MeGUI will ask how you want to open it and should give you the option to use DirectShow (unless it's an AVI in which case it'll want to use AVISource instead).
Of course you need to have an appropriate DirectShow decoder installed (ie something like ffdshow etc).
