This happens with Hi10P only?
Don't have much ideas either, make sure your GPU drivers and DirectX and madvr itself is up to date... Especially if you say it worked before.
Also just want to mention that while madvr is a good renderer, you're not getting much compared to any other renderer if Hi10P is your only concern. LAV does very good job dithering, not any worse than madvr.
Issue with mpc-hc not closing maybe bug in mpc-hc discovered after recent madvr change but this shouldn't affect playback.
Edit: just out of curiosity you can try this (http://forum.doom9.org/showthread.php?t=165890) and see if it makes any difference...
Edit2: i noticed you use LAV audio and video. enable LAV splitter too. and in MPC-HC filters page disable all source filters and decoders. You have MPC-splitters disabled for mkv/mp4 but does LAV spliter works when you play stuff?

This this (http://www.avsforum.com/t/1357375/advanced-mpc-hc-setup-guide) guide. If that doesn't work and VLC player doesn't either I guess you're out of luck.

I tried the guide no luck.
when i select Playback > Output > DirectShowVideo > madVR
MPC-HC completely stops responding to Play,Pause,Stop,Forward&Backward... nothing plays just blank screen.
when i close Player, it still remains in taskmanager so i had to use "End Process" to close it and run it again.
currently using "EVR (custom presenter)"
i searched a lot and doesn't seems to find an solution, i guess i'll give up on madVR
