If I check the MP4 box the videos just appear black and don't play
I find it strange that after so many revisions it still doesn't support MP4.... what gives? is it that hard to play MP4 files?

WorksForMe. Check your full DirectShow playback chain, I'm pretty sure whatever's making it not work isn't LAV itself.
(in MPC-HC the way to check your current filter chain is to check what's listed under Play->Filters)
And as a sidenote, yes -- the mp4 format is so robust implementing it is a pain. It is a mix of Apple's MOV and ISO committee's whims (they both remove and add things). Oh, and not to mention the specification is written in a way that's not exactly easy to read -- but not like the ISO Media Format specs are the only thing done like that.
A good example of the over-complication is that there is no specified way of doing chapters in the ISO Media Format. MOV had its own way, and later Nero implemented its completely own way of putting chapters in. Nowadays both can be found in mp4 files around the world (while now the MOV way is set not to be used, and the Nero way has never been standard), and what the committee tells one to do is to specify and implement something on top of BIFS.

It would be helpful if you could provide a MediaInfo file report for one of your problem .MP4 contained files?
