H.264 stream cutter

Is there some tool that can:
-cut an H.264 stream at any frame
-reencode frames at the beginning to fix open-gop/non-idr frame problems.
Does such a tool exist?

Just tested, seems to work fine and frame-precise, although the interface looks quite unprofessional, but unfortunately it cannot open raw H.264 files (don´t know why, it uses libav).
Any alternatives?
