I had recently downloaded a video from Film Riot's web site. The file appears to be an mp4 with h.264 video and AAC audio. The media info indicates that the file was created using mp4creator 1.5.0.1.

What was intriguing about this file is that it appeared to have its own custom icon that shows up in Windows Explorer.

When I say "custom icon", I don't mean that it's taking a thumbnail from the movie and using that as the file icon, but rather, it appears that a large (maybe 512x512) icon was embedded in the file and shows up in Windows Explorer.

I've tried doing a Google search for information on file icons for mp4 files, but I'm getting nowhere. Does anyone have any insight to this option?

Better to post screenshot of what it looks in explorer... however to confirm my guess I downloaded that video. So the thing you talking about is just image embedded as cover. You know the ones you use for music? This is it. Also from mediainfo:
Writing application : mp4creator 1.5.0.1
Writing library : Apple QuickTime
Cover : Yes
