I've finally solved the mystery of getting the ITV HD audio in sync.
Here's a picture of the complete mux including 10510 (ITV HD) in TS Packet Editor.

Obviously untick all the un-used PMTs just leaving 10510.
Then use "Edit->Table Options->Table Editor" to change the 0x0D49 (ITV HD V_PID) to "H264/AVC Video" and hit Update and Save.
Hit the Set (Start Point), move the progress bar to the end and hit the Set (End Point).
Change the drive and/or filename in the Save dialogue box (top left).
The important bit is to untick the Timecodes option in the bottom right and then hit the Edit button.
If you leave Timecodes ticked you won't hear any audio in the created TS file (I assume the audio is time shifted).
After this is finished Open the created TS file as normal - you'll now be able to view the video and hear the audio.
Edit out the adverts as normal and change the Save filename and hit Edit again.
You'll now have a TS file prepared for conversion.
Using the tsmuxerGUI method works but you have to guess what the audio delay is which isn't the best solution.
