how join 2 videos

I need a good tool to join mp4's (h264) + pcm audio track (possible with aac ?)

Is it like for instance divx: do they need the same bitrate. I say this because i have a hard time to obtain the same bitrate as my main video.(& I tried 2pass encoding) .

They need to be the same resolution and (video/audio) encoding settings. Bitrate, since you encoded with 2 pass VBR, shouldn't matter.
