I've written an updated version of the Premiere avisynth plugin which now works with Premiere Pro CS4. It also now works with After Effects and Soundbooth.

I've personally tested it against CS3/CS4 versions of Premiere, AE, and Soundbooth on WinXP and WinXP64. It should work with the CS2 versions as well, but I don't have any way to test that.
It seems a fair number of people cannot get it to work, but I'm not having any luck finding a pattern. It looks like in those cases, the plugin is file is being rejected outright before Premiere even tries to execute any code in it. Unfortunately, I still haven't been able to reproduce this problem on any computer I have access to. If anyone can reproduce this problem, please let me know as many details about your setup as possible. The symptom is that the filetype does not show up in the list when you try to import a file. It should show up as "Avisynth file (*.avs)".
I put up a slightly updated version (RC1) a few minutes ago, but the only real change between it and RC0 is that it was looking at the wrong spot in the registry to determine if it should stop audio conforming.
