I have a BluRay structure with menus and four TV episodes built through Nero9 - total size 4.34GB
Plays fine from the disk folder.
Plays fine on BD Player & PS3 when burnt on a BDRE (UDF2.5)
Does not play when burnt on a DVD-R (UDF2.5)
Any suggestions on where the issue might be?

Try to create "AVCHD DVD" with Nero. Not "Bluray Disk".

Never had a problem burning BlueRay structures created by TSMuxer (via RipBot).
Only difference is that instead of one set of mpls/clpi/m2ts files (created via TSMuxer) - there are multiple sets due to multiple clips and titles.

Ok - I had some more time to research - here is what I did
Source:
4 TV episodes captured off-air - edited with VideoRedoPlus to remove commercials, saved as MPG's.
Transcoded to 4 BluRay structures with RipBot to a target size of 1GB each.
Attempt 1:
NeroVision 9 - Created BluRay menu structure and burnt to HD Folder.
Used info from this post to fix index.bdmv and patch the index.bdmv and movieobject.bdmv (the 020 to 010).

With NeroBurningROM9 - burnt a DVD5 with UDF2.5 format and a BDRE with UDF2.5 format
Does Not Work on DVD5 - PS3 and Samsung 2500
Works on BDRE - PS3 and Samsung 2500
Attempt 2:
With DVD Architect5
A lot more complicated than the previous effort
Used TSMuxer to create mini (1 min) stubs of each of the 4 M2TS files from the 4 blu ray structures created in the source step
Then authored a new BluRay Menu with these 4 stubs and created an ISO image
Opened the ISO image and extracted the files
Replaced the 4 mini M2TS files in the STREAM folder with the corresponding full length M2TS files from the source step
Replaced the corresponding CLPI and MPLS files likewise
Patched the index.bdmv and movieobject.bdmv same as in first attempt.
With NeroBurningROM9 - burnt a DVD5 and BDRE with UDF2.5 format
This Works !! Menu's and all.
On Both PS3 and Samsung 2500
So what is so different in the files with Nero9 vs. the files from DVDA ?

I'm guessing you're trying to use AVCHD-Patcher as talked about in http://www.avsforum.com/avs-vb/showthread.php?p=15285018#post15285018 The programs I've used with AVCHD-Patcher are Encore and TotalMedia, and I think other people had reported using MovieFacory and DVDA. I don't think I've seen anyone else comment on using AVCHD-Patcher on a Nero Blu-ray output.
There two items I've seen previously mentioned that might exmplain why a Nero Blu-ray output might not work with AVCHD-Patcher. I've only used the Nero AVCHD output, and never bought the Blu-ray plugin, but I suppose Nero might include BD-J rather than being strictly HDMV. Another thing that I've seen MadMonkey57 mention as a possible issues is "HDMV menus with IG streams linked to an external M2TS via a so-called subpath mechanism", and I think that might also be what deank was talking about with "IGS". If you post the files, without the large m2ts in the BDMV/STREAM folder, then people could look at the files you're using. At this point my guess would just be that Nero maybe uses BD-J or the "subpath mechanism", and one of those could be a likely reason for why AVCHD-Patcher might not work with the Nero Blu-ray output.
