Please post a short (5 - 10 second) sample please. An image is less useful than a video clip to help you here.

Seeing the high amount of noise, I think you'll need something as strong as TemporalDegrain to prevent detail loss while doing aggressive filtering. As Sagekilla pointed out, a video sample is mandatory.
