DeSpot by Fizick is designed for exactly that problem

It can do miracles. Read his docmentation in the download for how to tweak the settings.

and the old RemoveDirt.dll filter can do some good stuff too.
(I haven't tried Kassandro's new script function version)

Be aware, getting rid of such large spots might make some blurring, and when I use DeSpot in motion scenes, it sometimes "eats" pieces of the scenery --- in a panning shot, some of the fence posts in the distance disappear and reappear, that kind of thing. So often I use JDL_ApplyRange to apply the DeSpot filter to just the few frames that really need it.
