I need help with Mpeg 4 AVC/AAC

Hi I have two files I think raw one with extension of .h264 the other is audio .AAC

How do I join them into one MP4 extension?
I want to play it on my PS3, its a 720p res video
Sorry I'm so noob & I dont get those tools all I did so far is extract this raw AVC/AAC & got stuck
