Hi,
Is it possible to demux an H.264 video stream and then mux it with custom subtitle file (like srt, or sub+idx) to a new file (ts/mkv) ?

Sure, and since mkvmerge has raw AVC support in the latest builds, you should be able to do it without much trouble. Have you tried?

I tried mkvmerge and the result was unplayable file (when it reaches to the point the subtitle needs to be displayed the player crashes). Also, when I opened the mkv file in graphedit it shows that the subtitle is displayed through DirectVobSub (VSFilter).

I nearly always put subtitles in my mkv encodes. I always use 'ass' though, and have never had a problem.
What format are you using?
If you mux the file without and put the subtitles with the same name with the file, what happens?
Also have you tried playing the file in VLC? VLC mangles the subtitles but has a go at displaying them.

Sounds more like you have a broken version of vsfilter than a bad file. Try updating it with the one in CCCP's current beta (see their site for info).
