This happened with the disc _The DaVinci Code_, but I'm pretty sure that it has occured on several other occasions and I just didn't make a point to notice it.
What happens is, when I first load the source directory, unreferenced anythings are not marked with a red X. If I then close and reopen RB, the same source directory is again opened, but the unreferenced anythings are now marked with a red X. Why this change in behavior?

It really doesn't matter. The unreferenced material isn't there -- so whether it is chosen for removal or it is kept, you get the exact same result.
I'll take a look at it and see if I can make the presentation more consistent for unreferenced tracks.

Though, when it had finished, I loaded RB's output as the source directory, and it still showed all the unreferenced material listed.

As it should. That's a function of the authoring package used on the original -- and DVD-RB recreates the backup as closely as possible to the original.

You can delete the unused streams in PgcEdit (http://www.videohelp.com/~r0lZ/pgcedit/index.html). Just right click on any title in a domain with unused streams, choose domain stream attributes and select delete for the ones you don't want to be referenced in VTSI_MAT.
