Unrecovered Read Error

Hello,
I am new @ making DVD's.
Most times I am successfull but lately I have been getting an Unrecovered Read Error.
While extracting sectors I get to about 4% complete and a I/O Error comes up after 20 re-tries. It gives me the option to skip a file and if I do then the DVD Shirnk cannot process the movie. Has anyone had this problem?
Anyone know how to fix it?
