muxing subtitles into an mpeg video stream

I was wondering if anyone knew of any methods to take an input stream(.mp4) from a camera and add a subtitle stream to it, then transmit for streaming again in realtime?

i've been play around with VLC, but it seems it is limited to only adding subtitle information to presaved video file and then streaming it over a network

If you want everything to happen in realtime, & if you can live with wmv, *I Think* there might be ways to do this, if I remember correctly from what I've read in the different winmedia sdks & docs.
