I have a DVD which is pretty bad out of sync (about -500ms).
The DVD contains a menu, some extras and several audio tracks.
Is there an easy way to fix the OOS? Or is there any way at all?

easy way out of this. Don't think so. A way out of this definetly.
U have to denux the dvd to audio and video and re-author it from the beginning, sliding the audio to its position. Good news r that u don't have to mess around with the menus cause if they don't suffer from the same disease, u can incorporate them as they r.
U can demux using vobedit.
For the reauthoring part u can use dvdlab www.dvdlabpro.com.It has a 30 day trial.
For incorporating the new vobs u can use ifoupdate and this mini guide (http://forum.doom9.org/showthread.php?s=&postid=550048&highlight=options#post550048).

First try, encountered 2 problems:
1. My original image was already reauthored a bit. 1 audio stream was blanked (Director's Commentary).
But this 2nd stream isn't demuxed by VobEdit.
2. I can't import the (about 10) .sup files into DVD-Lab Pro (latest beta). It seems it doesn't like that format.. :/
I guess I have to recreate the original dvd structured as exact as possible, what is the best way to do that after demuxing+fixing the delay?

1. of all u don't have to completly reauthor something because dvdlabpro doesn't accept the sup format.
Find what it wants and convert the sup to this format using subtitle workshop (http://www.urusoft.net/home.php?lang=1).
2. Director's commentary is missing. Replace it by a dummy audio file that doesn't need to be feature long. A couple of frames is enough. Trilight (http://dvdguides.trilight.net/) is sharing dummy audio files for ac3 and dts if u r intersted.

DVD-Lab Pro seems to want text-based subs (*.sub;*.srt;*.ssa;*.son;*.sst), and I only have the real subs from the dvd. I don't want to OCR them, I want to reuse them directly when muxing.
So I guess muxing with DVD-Lab is no longer an option.
The VTS I'm trying to recreate has 3 PGC's. I have all the streams ripped from those PGC's with DVD decrypter and I have fixed the sync problem.
Now I have to remux the whole thing.. Does anyone have a good suggestion for remuxing the streams?
1 VTS, 3 PGCs, each with: 1 video stream, 2 audio tracks, 8 subtitle streams

I don't know if this will help you or not, but I had a similar situation recently when I needed to replace pcm audio in a dvd with mp2. Here's the guide I used:

For your situation I'd just skip the audio conversion in besweet and, when re-authoring each vob in IfoEdit, adjust the audio offset so it will be in synch with the video.
All you need for the whole process is VobEdit, IfoEdit, and IfoUpdate. Well, and DVD Decryptor if the VIDEO_TS files aren't already on your hard drive, and a burning program if you want to get technical.
