If you are converting a DVD you have previously ripped you need to uncheck "Activate DVD ripping" in the DVD Rip tab.
Also, don't click on the disc icon in the Conversion tab, click on the folder tab next to it.
As regards menus etc, no. DVD2DVD only re-encodes the main feature not the full contents of the DVD. The easiest way to keep extras, menus and everything is to use one of the one-click transcoders like DVD Shrink or DVD2One. Admittedly the quality of the result is significantly lower.
It is possible to create a "hybrid" result using DVD Shrink to re-enc your extras etc and then use DVD2DVD for the main feature. You then author the DVD yourself so it is more complex. I haven't tried it but I know a user named Jsoto does this. If he's about he may help you out, if not do a search for his posts and see if you find anything helpful.

Nick
