Error Occurred While Reading From the Source Disc
If your source disc is scratched or dirty, your disc reader may not be able to read the media. Try the following actions to solve the problem:

Cleaning the disc
Remove all fingerprints, dirt, and dust from the shiny side of your disc using a nonabrasive cloth with a mild, nonabrasive soap solution.
Wipe the disc from the center to the edge of the disc.
Dry the disc completely.
Try using a different source drive
To change the drive you use to read your disc, click the Change button when asked to insert a source disc into your drive tray.
Select the drive you wish to use from the drop-down menu.
Insert your disc into the new drive.
Try using your recorder as both the source and destination drive
When you use your recorder to both read and write to your disc, RecordNow copies the entire disc to your hard drive and then copies that temporary file to your blank media. This can be helpful if the drive you're using to read your source disc is slow or unreliable.

Possibly because you are trying to read an encrypted disc and you haven't set the correct region code on your writer???

Same Situtation (AO5 & RecordNow) same problem. What type of media are you using? This is the first time I've tried to copy the backups I made on Ritek 03s. I get an error out around the edges when I try to make an image from the discs. This seems to be the same part of the disc that RecordNowMAx gives the dirty disc error on. Funny thing is I copy them on the fly in Nero and get no errors, they play fine in my standalone too.

I am using datawrite dvds.just did a test on audio and data the data failed first time ok second attemptaudio ok .Its strange using recordmax now i can use my dvd rom for reading audio and data cds to record with pioneer but dvds just get kicked out .I have to use pioneer for read and write. dvd rom reads dvds finre fine for powerdvd and windows media player.so only smarties have the answer. If there are no smarties then I will just record in the manner I am doing .At least I can get the required results.
