If you set the range of iframes to 2 (eg. min=2, max=4) then wouldnt "smooth quant fluc" become useless since its -/+2?

Nope - the smoothing function prevents the quantizer changes from subsequent frames from jumping > +- 2.
So, if you have an I-frame set to 3, and the codec decides the subsequent P-frame should get 10, the smoothing will jump in and set the P-frame to 5 instead.
