DVD BD sub question

Hi guys,
just 1 small question concerning subtitle.

I want to extract subtitle of from Prison break DVDs (e.g. slovenian) and add it to the same series but in BD Full HD format, cause they don't exist in that format, they are only present on DVDs.

In which format should I extract them from full dvds (most suitable for adding after to BD menu) ?
Which tool is most recommended to use ?
Which tool to inject/add them to BDMV structure (placing on the place of someone else sub) ?

Rip the DVD subtitles as IDX/SUB using VobSub, then use BDSup2Sup to convert them from IDX to SUP (and resize them to 1080p, and adjust the framerate to 23.976), after that use MultiAVCHD to add the new subtitles to the Blu-ray (you can replace an already existing subtitle stream or create a new one).
