Pgcdemux can extract whole stream only...
To extract forced only use e.g Subrip (http://www.videohelp.com/tools/Subrip) (Options - Global Options - Forced subtitles only)

It's as easy as pie using DVD SubEdit - and selecting "Only show forced subpics"...
