What % is your CPU at during playback? H264 encodes vary a lot in decoding requirements, some are worse than Bluray, some you could play on a PocketPC.

What renderer are you using in MPC? Try Overlay Mixer, it's usually the fastest.
Where did the source come from / what settings was it encoded with?

I am having a similar issue except it isn't the video playing choppy, it is occasional tearing of the image when played on a 92 inch screen. It isn't really noticeable when played on a monitor. I am using VMR9 because I seem to be unable to view subtitles when using overlay mixer. The overlay mixer renders the video better but it can't display the subs. Is there a way around this?

You need a video app that supports exclusive mode in VMR9: Zoomplayer or Theatertek are the only ones I know. Only guaranteed way to get rid of vmr tearing.

Well Then maybe a better question would be is there a way to show subtitles with the overlay mixer?
