You will only be able to retrieve them at VCD resolution.

Hi!
1) Open the MPEGAV folder and copy the "AVSEQ01.DAT" file onto your harddrive
2) Load it into TMPGEnc
3) From the "File" dropdown menu, select "Output to file" - and choose "Sequence BMP/PPM/TGA/JPG file"
4) Enter a new filename and (my preference) choose JPEG output format

on *nix i've used a program called vcdgear (http://www.vcdgear.com/faq.html) to extract useable data from vcds, seems to handle
the lack of error correction and the vcd "psuedo filesystem" fairly well.
I would imagine there is a port available for most OS's.
There probably is something better, but that's the first thing to
come to mind.

1) Open the MPEGAV folder and copy the "AVSEQ01.DAT" file onto your harddriveNot necessarily. It might depend on the particular version of Nero used, but at least e.g. Nero 5.5.x versions put stills in to the segment folder where you can find them as ITEMxxxx.DAT files. These dat files actually contain both high- and low-res versions of the still image (VCD supports full D1 resolutions for stills).
To extract both resolutions, load the dat file in the TMPGEnc MPEG Tools->De-Multiplex. You will see both resolutions as separate streams, double-click to extract either of them.
Afterwards, continue with item 2 on setarip_old's list. vcdgear has not worked for me with mpeg stills.

yep, let's hope the stills are D1 cropped (704x576 or 480)

At least Nero 5.5.x includes both 704x576/480 and 352x288/240 in the stills - I tried it for the first time before posting a reply :).
