Verify the FPS on the H264 file.
If the FPS on the H264 file is different from the DVD it cause problem on video and audio syncronization.
Demux your encoded file to raw .264 and use the MP4 Muxer inside the software MeGUI and set the correct FPS.

use yamb for the remuxing (make sure to set the correct fps!)

http://img231.imageshack.us/img231/1786/extractraw2if.jpg <<-- open Yamb 1.5 go to Extract Tab select the file them clic on "extract track ID 1 in raw format" and clic on the Extract button the program will create a .h264 file on the same folder of the original file.
(as you can see on the image Track 1 is the video track)
I dont found on Yamb 1.5 the option to choose FPS.
Rename the .h264 to .264 and open MeGUI
http://img208.imageshack.us/img208/6090/mp4muxer010lg.jpg <<-- select Mp4 Muxer
http://img211.imageshack.us/img211/5718/mp4muxer020dq.jpg <<-- input the raw 264 file and choose the correct FPS.

some left about that.In my case, when encoding to h264,TS mux and mpeg format for saving file.So is it any problem when using yamb 1.5.
Because I saw follow log
"Failed to open MPEG file C:\wujiandao.mpg
Error probing file C:\wujiandao.mpg: BitStream Not Compliant
(BitStream Not Compliant)
(BitStream Not Compliant)
"
from yamb

philex, mp4box doesnt support avc in .ts or .mpg as input
encode with mainconcept to raw .264 (es) and mux this into .mp4
now that you have encoded the stream already, extract the raw stream from your .ts or .mpg with mplayer
