Use YAMB to put the RAW .264 file into an .mp4 container. Than input this .mp4 container into MKVMerge and you'll have a working .mkv file.

it's the default YAMB settings, make sure you set it to 23.97 before muxing to mp4

When mux to mkv, just add a timecode file, which set the fps to 23.97.
Such as:
# timecode format v1
assume 23.976

Side question...
What kind of things can you do with a RAW x264 stream that you can't do with a MP4/MKV one?

You can use the H264_parse tool to see if the stream was encoded correctly. You may be able to do other things too.
