How to get chapter titles?

I use DVD Decrypter to extract chapter information which I put into my MP4's using YAMB. The chapters all have generic names like Chapter01, Chapter02, etc. I know I can go in by hand and edit these, but I was wondering if there is an automated way to get the chapter titles?

My Windows MCE 2005 box displays chapter titles when watching a DVD, so the textual information exists somewhere on the DVD, I just don't know where.

ChapterGrabber loads chapter names from websites. I would really like to know where the chapter names are stored on DVD and why not extract from there.
